At Mayo Clinic in Rochester, the effort to reduce readmissions is evolving. It started with a highly successful pilot program two years ago in the Division of Hospital Internal Medicine. The small-scale pilot, initiated by division chief David Klocke, M.D., began by teaching physicians which patients were at high risk for readmission and helping identify the causes of readmission, such as a patient not understanding exactly how to take a medication.
